Inadvertent leak at Verizon may expose 63k of its employees

I saw this earlier today, and am now getting a chance to blog this.

The article comes from Cybernews and is titled Verizon employee inadvertently leaks data of 63 thousand colleagues | Cybernews which talks about the matter. My question would be, how did they get access to said file in the first place? Was it stored on a server somewhere that they could exfiltrate if they so chose? What about the rest of the 100k plus employees who were not affected by this? And the biggest question, what is Verizon doing about making sure that this accidental leak doesn’t happen again? Finally, who was this supposed file leaked to?
